this is not really a plugin question.
But in general: the same as in usual clearnet.
Use the websearch as usual for forms and include them in your webpage.

Your answer was not at all helpful for a new comer like me. I set up my eepsite and want to add a contact form in that. I just wanted to know how it is possible. Is php a good option?

As we we are in te plugin section of the forum, thats not really a plugin topic neither plugin question.
And the answer stands: do it as every other webpage in the internet does it.
Ask the folks who are doing webpages (and not the I2P plugin creators) to get more in detail answers, as we do not know that topic.
Also you may search the internet with the usual tools to find a solution working for you.
If you want to use the builtin jetty server, you would need to dig a bit deeper to active scripts, as Jetty is slightly different.
But you can also use a usual apache2 or any other webserver you like.
